WHAT IS HOME HEALTH CARE?

Home Health Care is defined as home visits made by health care professionals and workers including nurses, nurse practitioners, and personal support workers (PSWs). Care is provided to individuals in their place of residence to ensure long term health and vitality. My Place Home Care offers non-medical services provided by PSWs and companion caregivers. Services are tailored to our clients’ needs and may include, companionship, housekeeping, assistance with activities of daily living such as feeding, toileting, selecting proper attire, grooming, maintaining continence, help getting dressed, bathing/showering, walking and transferring, and instrumental activities of daily living such as cooking, meal planning, transportation to and from appointments, shopping or running errands, and medication reminders and management.

WHAT IS A PSW AND WHAT ARE THEIR QUALIFICATIONS?

PSW is short for personal support worker. They are front line health care professionals in Ontario that provide direct care to the elderly or ill patients in a wide variety of settings. “PSW” is a generic title provided to an individual, they are also known as health care aides, personal attendant, visiting homemakers, respite care worker, home support worker, palliative care worker, supportive care assistant and patient services associate. A PSW is the person who helps an individual do the things that the person would do for themselves if they were physically and or cognitively able. There is no standard scope of practice. Some of the most common roles and duties associated with a PSW are: providing assistance with Activities of Daily Living (ADL), providing assistance with Instrumental Activities of Daily Living (IADL), medication reminders, transportation for shopping and running errands and accompanying a client to an appointment and taking notes. My Place Home Care PSWs hold a Certificate in Personal Support Work (or equivalent) obtained from a community or private college. PSWs are trained to deal with families and their loved ones in any setting and with a variety of conditions including Alzheimer’s, dementia, and palliative care.

How do you select your PSWs and Companions?

Each personal support worker (PSW) and companion/caregiver is carefully screened and selected based on recommendations from our HR department, our HR Manager has over 20 years’ experience working in home care. Each person who applies goes through our rigorous screening and interviewing process, and personal and professional reference checks. Our Companions are all experienced individuals from a variety of backgrounds, such as retired public service workers, business people, nurses, PSWs, RPNs and teachers. They are carefully selected based on their experience, reliability and their willingness to help families get the most out of life and to provide the highest quality of living to everyone.

What if I don’t like my PSW/Companion?

We want our clients to be happy, healthy and ensure that every Personal Support Worker or Companion visit is a great experience.

That’s why after the first few visits you will be contacted by a My Place Home Care representative to establish how we are doing. For any reason , if you feel the Personal Support Worker or Companion is not the best fit for your loved one we will introduce you to new caregiver. We will continue until we find the best fit for your loved one.

Will I have the same caregiver all the time?

Continuity of service is important, and so our aim is to create long term caring relationships between our caregivers and clients.

To build continuity, we introduce our clients to more than one caregiver with a view to selecting a main cargiver and a back-up caregiver.

What is an LHIN?

LHIN, stands for Local Health Integration Network, the Ottawa area is served by the Champlain LHIN. The aim of the LHIN is to help coordinate health services so that people receive the care they need and deserve in a timely manner. The LHIN does not provide services directly. It is their mandate to ensure that services are well organised, appropriately funded and meet the needs of all residents in their area.

Do you only assist seniors?

Our services are available for any adult looking for any sort of assistance with Activities of Daily Living and Instrumental Activities of Daily Living or just joyful companionship.

Why should I choose My Place Home Care over another agency?

Our process starts with the client and their family – through a personal interview we get to know the client, their personality, likes, dislikes, medical status, emotional status, and budget. In some cases, family members may live in another city or not be available to visit their loved one as frequently as they would like. All My Place Home Care personal support workers are formally trained and certified to practice in Ontario. My Place Home Care helps personal support workers extend their skills through periodic specialised training. We strive for continuous quality improvement to be able to offer the highest quality service to our clients and ensure we improve each time we visit.

Do I get to set my scheduled visits?

My Place Home Care delivers home care the way you want it. You choose – morning, afternoon, or evening. Your schedule is our schedule. Clients control how often we visit and the length of our visit.

Is there a fee for the initial consultation with My Place Home Care?

No. My Place Home Care offers a free no obligation consultation in the comfort of your home. The consultation allows us to get to know you and share information about available care options.

Are you able to care for individuals with who are experiencing symptoms of dementia, Alzheimer’s disease or other cognitive impairments?

My Place Home Care Specialized Personal Support Workers (PSWs) are available with experience working with dementia and Alzheimer’s patients. We will even design a personalised day program for your loved one which will use a variety of therapies and activities that ensure complete engagement throughout our visit.

What is companion care?

Companion care is primarily emotional support and companionship for seniors who are generally healthy and want to stay independent in their own homes.

Companion care is what the name implies. It can be as simple as someone to build a relationship with to engage in these activities and more:
share stories
go out for walks
play board games
play cards
provide medication reminders
light housekeeping duties
meal planning
meal preparation together
planning
scheduling and keeping appointments
transportation assistance to the grocery store, running errands, medical appointments
planning social activities including movies, shopping, visiting family and friends, exercising
Staying in touch with family using the latest communication technology.\

What is a Care Plan?

A care plan is a document that defines an individual’s personal care needs and services.

In particular, it outlines:
The required personal care services,
The type of staff that will provide the service,
how often the service is needed – the kind of required equipment or supplies needed,
Special diet or food preferences,
Client health and personal goals,
Information on how to use a care plan to achieve the goals.

The Care Plan also outlines the roles and responsibilities of the parties involved in a person’s care.

ARE THERE ANY SUBSIDIES OR TAX BREAKS AVAILABLE FOR HOME HEALTH CARE?

The quick answer is yes, there are many subsidies that can fully or partially cover your home health care services. As an example, the Veterans Independence Program (VIP) – VIP can help pay for services such as grounds keeping, housekeeping, personal care, access to nutrition, health and support services provided by a health professional. There are also tax breaks when it comes to home care. Give us a call and we will discuss the subsidies that best suit your situation. That’s why after the first two visits you’ll be contacted by a My Place Home Care representative to establish how we’re doing. For any reason , if you feel the Personal Support Worker or Companion is not the best fit for your loved one we will introduce you to new caregiver. We’ll continue until we find the best fit for your loved one.

WHAT IS THE DIFFERENCE BETWEEN ACTIVITIES OF DAILY LIVING (ADL) AND INSTRUMENTAL ACTIVITIES OF DAILY LIVING (IADL)?

Activities of daily living are things that are necessary for fundamental functioning, such as feeding, toileting, dressing, grooming, bathing/showering maintaining continence and transferring. Instrumental activities of daily living are not necessary for fundamental functioning, but they allow an individual to live independently in a community, things such as housework, preparing meals, transportation, shopping keeping track of finances, and medication reminders and management. At My Place Home Care, we want to work with families to ensure that family members and their loved ones are receiving the best care possible so as to promote independence, good health and vitality.
