NURSING SERVICES
My Place Homecare understands that illness, disease, or any situation that has changed the normal in your life, often creates some unknown in your space. These situations may require teaching, treatments, procedures, and follow-up.
The nurses that work alongside MPHC can offer clients assistance and care in many areas along the healthcare spectrum. This includes, but is not limited to:
- Care Plan building to meet goals and needs
- Mini Cognitive Assessments
- Medication Management
- Navigating the healthcare system
- Consulting to identify pertinent referrals and resources
- Assistance and teaching to complete Advanced Care Plans
- Assessments and Reassessments with changes in status, and recognizing changes in needs and goals
- Dressing changes and wound care
- Home-ordered intravenous infusions
- Client and family teaching that includes, but is not limited to:
- Medications
- Safety in the home
- Lifts and other mobility equipment and skills
- What might be seen throughout the stages of Dementia
- What might be seen throughout the stages of a life-limiting conditions
- Assisting families with conversations relating to wishes and goals relating to care

Nursing Replacement Shifts
Nurses can be contracted out for replacement shifts to cover the DOC, ADOC, RN, and RPN.
