Comprehensive Home Care Services
CONVALESCING SERVICES
We work with the discharge staff after a hospital or rehabilitation stay to provide services including medication management, assistance with meal preparation, housekeeping and running errands.
PERSONAL CARE SERVICES
Personal Care Services consist of activities of daily living such as bathing, washing, grooming, dressing, oral hygiene, mobility assistance, transferring and positionin. We offer 24-hour service, 365 days per year.
DEMENTIA CARE
At My Place Home Care, our Positive Approach to Care (PAC) certified nurse teaches our caregivers advanced techniques and skills to properly understand the different behaviours seniors with dementia may experience.
PALLIATIVE AND END OF LIFE CARE
This can include personal care, meal preparation, light housekeeping, overnight care, and companionship to provide respite for caregivers.
FOOT CARE SERVICES
For non-high-risk patients, our footcare certified PSW’s offer in-home assessment, clipping and filing of toenails, ingrown toenails and foot soak and basic foot care.
RESPITE CARE
Our staff can assist families by giving them a much-needed rest through our respite care services. You decide how long and how much support you need from caregiving, and we provide support and peace of mind.
HOME MEDICAL EQUIPMENT & SUPPLIES
We provide advice and support in obtaining home medical equipment such as walkers, wheelchairs, scooters, canes, grab bars, commodes, toilet safety products, incontinence products, shower and bath.
COMPANIONSHIP AND COMPANION CARE
Providing engaging conversation and companionship services to seniors is essential and can reduce the effects of loneliness and social isolation.
Our Approach
At My Place Home Care, our purpose is to provide quality home care services to seniors and persons with disabilities so they can maintain their independence for as long as possible. Aging in place is a newer phenomenon many seniors are leaning towards in their golden years and home care plays a critical role. Instead of moving to a retirement community, seniors are opting to stay in their own homes and community home care helps to make this possible.
My Place Home Care aims to support seniors and their families to help you live your best life in a safe and comfortable environment. The care process with My Place Home Care can be broken down into 3 easy steps:
Step 1: Free in-home care assessment
The goal of the assessment is to further understand the needs and wants of each individual and their families. Over the 60–90-minute intake meeting we will develop a comprehensive and thorough care plan tailored to the needs and wants of our client. The meeting will include in-depth questions and discussion to ensure all facets of the client’s care is covered
Step 2: Development of your personalized care plan
Using the information gathered during the free care assessment, our team will prepare a care plan unique to the client and family needs and wishes. The care plan integrates client requests but also enables us to take the time to advocate for you and your health needs. We work hard to understand the circumstances facing our clients and their loved ones and create a care plan customized to them.
Step 3: Ongoing follow-up
We strive for continuous quality improvement while we care for you or your loved one. We achieve this through our quality checks. Our management team will follow up regularly with you or your loved one regarding the quality and consistency of care being provided through follow up phone calls, emails, and/or visits while the caregiver is providing services.
In addition to providing quality home care, our clients can take advantage of our extensive care network to ensure all their needs are met. We can provide additional information and referrals for various services including:
- Mobile hair care
- Physiotherapy
- Occupational therapy
- Diabetic foot care
- Home medical equipment and supplies
