My Place Home Care provides a free in-home assessment that lets us get to know our client and their family. The assessment helps us understand what services are required so we can develop a care plan. In addition to discussing what you want, we also take the time to advocate for you and your health needs. We work hard to understand the circumstances facing our clients and their loved ones.
Our management team also conducts regular supervisory visits and makes follow-up phone calls to ensure that the care plan is working for the client and their family.
Personal Care Services consist of activities of daily living such as bathing, washing, grooming, dressing, oral hygiene, mobility assistance, transferring and positioning, toileting and incontinence care, medication reminders, and assistance with eating. We offer 24-hour service, 365 days per year.
Together with conversation and companionship, we help with meal preparation, laundry, light housekeeping, grocery shopping, errands, transportation, social planning, and medication reminders. We provide 24-hour services if needed.
To maximize our clients’ quality of life and independence we offer companionship, and personal care services, as well as physical and mental activities such as walks, range of motion training, puzzles, storytelling, reminiscing, and card games. This can include personal care, meal preparation, light housekeeping, overnight care, and companionship to provide respite for caregivers.
My Place Home Care provides transportation to appointments, grocery shopping, social gathering, or any other event where personal transportation is required. Upon request, our caregivers will not only bring the client to his or her appointment, they will stay with them to take notes. For errands or grocery shopping, our caregivers stay with the client to assist as needed.
My Place Home Care provides care to clients after a hospital or rehabilitation stay. Services provided include working with the discharge planning staff to create and support the care plan to aid the recovery process; medication management and assistance; assistance with household activities and activities of daily living such as meal preparation, housekeeping and running errands; follow-up physician visits, including reminders and transportation services; and communication with the medical team as needed to ensure peace of mind for our clients and their family members. .
For non-high risk patients. Specific services offered include in-home assessment, clipping and filing of toenails, ingrown toenails and foot soak and basic foot care. All services are provided by a PSW certified in basic foot care. If you are a high-risk patient, we can recommend a service just for you.
My Place Home Care can assist families by giving them a much needed rest through our respite care services. It is nice to know that a compassionate, considerate person can provide the primary caregiver a break in order to recharge, run errands, go to an appointment, visit family and friends, and or go on vacation. We will help you get that peace of mind knowing someone you and your loved one have chosen is providing the support you need. A familiar friendly face will always be there to help when you need them. My Place Home Care caregivers are available for a few hours, daily, weekly, and overnight. You decide how long and how often.
