CONVALESCING SERVICES
For people recovering from surgery, returning home from a long hospital stay, or being discharged from the hospital for other reasons, convalescent care services are a great option to help individuals recover and recuperate with specialized care. My Place Home Care provides convalescent care services to clients after hospitalization or rehabilitation stays enabling them to recover faster, better, and more safely the one would alone.
Our Convalescing Services include:
- Working with the discharge planning staff to create and support the care plan to aid the recovery process
- Medication management and assistance
- Assistance with household activities and activities of daily living such as:
- meal preparation
- housekeeping
- running errands
- follow-up physician visits, including reminders and transportation services.
- communication with the medical team
